You are invited to participate in this online research survey entitled “How Socioeconomic and Health Risk Factors Impact the Fertility Decision Making Process.” You are included in this survey because you have experienced issues with fertility and sought in-vitro fertilization (IVF) and/or intrauterine insemination (IUI) as a result at some point in your medical history. The purpose of this research study is to examine the different factors that influence decision making when pursuing fertility care, specifically in regards to in-vitro fertilization and intrauterine insemination. The number of subjects to be enrolled in the study will be approximately 375.
The survey may take approximately 10-15 minutes to complete. Your participation is voluntary. If you do not wish to participate in this study, do not respond to this online survey. There are no anticipated risks or discomforts associated with completing this survey. There may be no direct benefit to you, however, by participating in this study, you may help to provide insights into future approaches for counseling to address the specific needs of patients seeking fertility care. The number of subjects to be enrolled in the study will be 375.
Your response will be kept confidential. All collected data will be stored in a secure computer file and the file will be destroyed once the data has been published. Any part of the research that is published as part of this study will not include any identifying information. Participants must be 18 years or older and have completed fertility treatment via in-vitro fertilization (IVF) and/or intrauterine insemination (IUI) in the United States in the past. Completing this survey indicates that you are voluntarily providing consent to participate in the survey.
