Participation is voluntary. To protect your decision of whether to participate, participants unwilling to complete the survey are asked to turn in a blank survey. Completed services will also serve as a participant’s consent. All surveys will be turned in to a member of the research team.

Section 1 - Please complete the following scale by identifying for each item your ideal rating versus the actual rating you see occurring in your school. Please rate each item from 1 (Strongly agree) to 5 (Strongly disagree). 1 The term practitioner* means anyone (i.e. school psychologist, counselor, teacher, para-educator, therapists, and others) who provides services to students. 2 - The term teacher** is used to identify classroom teachers as well as special educators, teachers of bilingual students and others who provide direct instruction to students.
